What to Bring to your Appointment:

  1. Driver’s Medical Examination Report (DMER): The physical form provided by the licensing body (e.g., RoadSafetyBC or your province's ministry of transportation).

  2. Photo Identification: A valid driver's license, passport, or provincial ID.

  3. Provincial Health Card: For identification and records.

  4. Corrective Lenses: Glasses or contact lenses if you wear them.

  5. Medication List: A list of all prescription and over-the-counter medications you are currently taking.

  6. Medical Information: Reports or test results for existing medical conditions if asked by the doctor.
